New Patients At Nourishing Medicine

If you are thinking about making an appointment, I offer a complimentary consultation so that you can meet me and see my office. This consultation is designed to give prospective patients an opportunity to meet me, see my treatment space, and learn about my philosophy of health. I explain the benefits of Chinese medicine as a system of healing and health maintenance. This consultation does not include diagnosis or specific health care advice but instead is focused on education and allowing time for the prospective patient to interview Elie Cole as a practitioner.

If you have already made an appointment, I look forward to meeting you.  First, please download and print out the following two forms.  Spend some time to fill out the health history form.  You can sign the consent form which we will discuss together.  Second, there are a few things I would like you to read before coming in. In the drop down menu above under new patients, please read both Frequently Asked Questions under the New Patients tab as well as Acupuncture under the Services tab. Thanks!
